FOR DEATH OCCURRING IN COLORADO:
Pre-Burial
- The funeral director or person acting as such shall complete a Delta County Request for Private Burial form and a Death Registration Worksheet and return these completed forms to the local office of vital statistics:
- County Clerk and Recorder (Delta), 501 Palmer St. #211, Delta, CO, 970-874-2150. Open Monday through Friday, 8:00 am to 4:30 pm.
- County Clerk and Recorder (North Fork Annex), 196 W. Hotchkiss Ave. Hotchkiss, CO 81419, 970-872-1207. Open Monday through Friday, 8:30 am to 12:30 pm, and 1:30 pm to 4:30 pm .
- County Coroner (if Clerk’s office is not open), 682A 1725 Road, Delta, CO, 970-874-5918. Available 24/7. (Calls outside of business hours go through County dispatch.).
For Burial
- The local office of vital statistics will issue a Disposition Permit and an Affidavit of Private Burial form to the funeral director or person acting as such.
- The funeral director or person acting as such completes the bottom portion of the Disposition Permit along with the Affidavit of Private Burial as required by Colorado Revised Statute 25-2-111 to the local office of vital statistics.
Post-Burial
- The Disposition Permit will need to be signed by a deputy registrar and the Affidavit of Private Burial will be recorded into the records of the Delta County Clerk and Recorder. There is a recording fee of $43.00 when filing the Affidavit of Private Burial.
- The funeral director or person acting as such will be given the signed Disposition Permit and the recorded Affidavit of Private Burial for the land owner’s records.
NOTE: The funeral director or person acting as such is responsible for ensuring the remains are properly embalmed or refrigerated if disposition does not occur within 24 hours.
